MAULDIN HS ATHLETE ELIGIBILITY

Mauldin High School will sponsor a Girls and Boys’ Swim Team for the 2010-2011 year and will be open to students, currently attending MHS or a 7^th or 8^th grader at a middle school that feeds into Mauldin High School.  Each athlete will need to become high school eligible prior to the try-outs for the season.  The requirements are as follows:

          1) INFORMATION SHEET (on swim website)

2) PHYSICAL FORM (2 pages on school website)

3) GHS CONSENT FORM for medical treatment (2 pages on school website.)

          4) MHS EXTRA-CURRICULAR RESPONSIBILITY ACKNOWLEDGEMENT FORM (on school website)

5) ORIGINAL BIRTH CERTIFICATE seen by Coach Gettys and a copy of it for me to file.  (If you have already participated in a high school sport, I can check to see if it is on file.)

          6) ATHLETIC INSURANCE--$36 (subject to change) Make checks payable to Mauldin High School.

          7) STUDENT VEHICLE USE FORM (2 pages on school website)

          8) ACADEMICALLY ELIGIBLE as outlined by Mauldin HS and SCHSL.        

Forms in #2, #3, #4, and #7 must be signed by BOTH parent and athlete.

Final Deadline for swim team forms—July 27

TRY—OUTS:

July 29                        MIDNIGHT-12:00am—2:00am

July 30                        7:30pm – 9:45pm

August 2-3                 7:30am—9:45am

Regular swim meets will begin the week of August 16.

Region Championship—September 25

State Meet—October 9
