MAULDIN HS ATHLETE ELIGIBILITY FORMS
Mauldin High School will sponsor a Girls and Boys’ Swim Team for the 2009-2010 year and will be open to students, currently attending MHS or a 7th or 8th grader at a middle school which feeds into Mauldin High School. Each athlete will need to become high school eligible prior to the try-outs for the season. The requirements are as follows:
1) 2009 INFORMATION SHEET
Must be mailed or dropped off ASAP to Coach Patricia Gettys or parent volunteer Tina DeLaney.
Note: Grade is for next year. Please print legibly especially email addresses as this is our main means of communciation.
You must download the following forms from school website: www.greenville.k12.sc.us/mauldinh/
2) PHYSICAL FORM (1 page front and back)
3) GHS RELEASE FORM for medical treatment (1 page front and back )
4) MHS EXTRA-CURRICULAR FORM (1 page)
Forms in #2, #3, and #4 must be signed by BOTH parent and athlete.
5) ORIGINAL BIRTH CERTIFICATE seen by Coach Gettys and a copy of it for me to file. (If you have already participated in a high school sport, I can check to see if it is on file.)
6) ATHLETIC INSURANCE--$36 (note this increase) Make checks payable to Mauldin High School.
7) ACADEMICALLY ELIGIBLE as outlined by Mauldin HS and SCHSL. (www.greenville.k12.sc.us/mauldinh/ )
